Patients & Course Models, and by law are required to fill out the necessary consent forms prior to treatment. Please have these forms filled out prior to coming to your appointment as they are required to be reviewed by a medical professional. We cannot treat any patient or Model who arrives without completed consent forms.

ALL Patients & Models MUST fill out the New Patient or New Model Registration Form + ALL applicable treatment forms that pertain to your services only.
